Can You Take Ibuprofen and Spironolactone Together?

What the interaction means

Spironolactone's FDA prescribing information has a section titled "7.3 Nonsteroidal Anti-Inflammatory Drugs (NSAIDs)": "In some patients, the administration of an NSAID can reduce the diuretic, natriuretic, and antihypertensive effect of diuretics... when spironolactone tablets and NSAIDs are used concomitantly, monitor closely." Section 7.1, "Drugs and Supplements Increasing Serum Potassium," also lists NSAIDs. Ibuprofen's own FDA label corroborates from the ibuprofen side: "Ibuprofen tablets can reduce the natriuretic effect of furosemide and thiazides in some patients, a response attributed to inhibition of renal prostaglandin synthesis."

Why it may happen

NSAIDs like ibuprofen inhibit renal prostaglandin synthesis, which can blunt spironolactone's diuretic, natriuretic, and antihypertensive effects and, because spironolactone is also potassium-sparing, contribute to elevated serum potassium.

Practical context

Spironolactone's label explicitly recommends monitoring closely when NSAIDs and spironolactone are used together. Anyone taking spironolactone for blood pressure, fluid management, or hormonal purposes who also uses ibuprofen regularly should discuss this with the prescriber, since both reduced spironolactone efficacy and elevated potassium are possible.
