What the interaction means

Lisinopril's FDA prescribing information, Section 7.3, "Non-Steroidal Anti-Inflammatory Agents Including Selective Cyclooxygenase-2 Inhibitors," states: "coadministration of NSAIDs, including selective COX-2 inhibitors, with ACE inhibitors, including lisinopril, may result in deterioration of renal function, including possible acute renal failure... The antihypertensive effect of ACE inhibitors, including lisinopril, may be attenuated by NSAIDs." Ibuprofen's own FDA label corroborates: "NSAIDs may diminish the antihypertensive effect of ACE-inhibitors. This interaction should be given consideration in patients taking NSAIDs concomitantly with ACE-inhibitors."

Why it may happen

NSAIDs like ibuprofen inhibit renal prostaglandin synthesis, which can reduce glomerular filtration and blunt the blood-pressure-lowering effect of ACE inhibitors like lisinopril, particularly in patients who are elderly, volume-depleted, or have compromised renal function.

This is most relevant to regular or higher-dose ibuprofen use rather than a single occasional dose, and especially relevant in elderly patients, those with reduced kidney function, or those also on diuretics.
